- Vena contracta > 0.7cm
- PISA radius > 1cm
- Regurgitant fraction > 50%
- Regurgitant volume > 60 ml/beat
- Regurgitant jet occupies > 40% left atrium
- Regurgitant jet is large
- Pulmonary systolic vein flow reversal is seen
- Left heart is dilated
- The mitral valve is rarely normal - evidence of a ruptured papillary muscle or a flail leaflet
- E-wave velocity >1.5 m/s
